    URSULINE ACADEMY
    WORK PERMIT INSTRUCTIONS

    For Students Under The Age of 18


    To obtain a work permit, please follow these steps.

    1. Obtain the 2 pre-application forms
      a. an Application for Minor Work Permit which also includes the Pledge of the Employer Form

      b. a Physician Certificate for Minor Work Permit Form
    Note: You must provide either a completed & signed Physician’s Certificate or a copy of a valid physical with physician’s signature.

    Students may pick these forms up inside the guidance office or you may download them at: www.com.state.oh.us/ODOC/. Click on the Work Permits box and scroll down & click on Minor Work Permit Pre-Application Form (PDF) and also Physician’s Certificate (PDF). Students may also go to Edline and click on work permits.

    2. A certified copy or original Birth Certificate is already on file at Ursuline as part of your original registration, so there is no need for further proof.

    3. The pre-application form(s) must then be completed in entirety by each party and signed. The Employer’s Tax ID Number is mandatory.

    4. The student should return the completed forms to the guidance office. The designated school person will issue the Minor Work Permit within a week. The student should return in a couple of days to sign the permit in the presence of a school official and then, must take the permit to the employer.

    REMEMBER: For these forms the school district is: The Archdiocese of Cincinnati and the school building is Ursuline Academy.
