School
Nurse
The primary role of the school nurse is to serve as the liaison
between education and health. We strive to promote, facilitate, and
maintain health and wellness in the entire school community. The school
nurse does not diagnose a condition, prescribe medications or make
decisions related to medical plans of care. Please contact us with any

Medication
Policy
NEW!
“All prescription medications must come to school in the original
container from the pharmacist.
The Nurse’s office will supply Acetaminophen 325mg tablets and
Ibuprofen 200 mg tablets with a completed medication form that is
signed by a physician AND parent. All OTHER over-the-counter
medications must be supplied by parent in the original container and
accompany a medication form that is signed by physician and parent.”
The following regulations are in effect for ALL prescription and
over-the-counter medications.
- All prescription medications must be brought to school by a
parent or delegated responsible person. The medication needs to be in
its original container from the pharmacist.
- A Medication Form must be filled out completely indicating the name
of the medication, dosage, time, and duration. This form must be signed
by both the parent and the physician. A new form must be submitted each
year to the school nurse.
- Students may carry an inhaler if the inhaler section of the
medication form is filled out and the parent/physician confirms that the
student may carry her inhaler.
- EPIPEN users must fill out a separate form and submit it to the
Nurse’s office. It is required that the student must carry one EPIPEN
and have a second EPIPEN kept in the Nurse’s office.
- All forms can be obtained through the Nurse’s office, or click here
to download the forms. Water and food passes must be obtained through
the front office or Nurse’s office.

Screening
Program
All freshman students are screened for vision, hearing, and postural
problems. The screening takes place in the fall, winter, and spring
quarters. Parents are notified by phone or mail if the student does not
pass a particular screening. If the student is already under the care of a
physician for scoliosis and the parent does not want her to be screened,
please let the Nurse’s office know.
Absence
As stated in the student/parent handbook, parents should report
absences by calling the attendance line on the day of the absence. In
addition, students should bring a note to the main attendance office
explaining ALL absences on the day they return to school.
Nutrition
Here is a great link to a website that is sponsored by our food
service provider, Sodexo. It has valuable nutrition information about the
foods that we serve in the café. There are also some nice menus for
milk free diets, gluten free diets, peanut free diets, etc… Many new items
have been added to the café. Check out the yogurt cups, turkey sandwiches,
and fresh fruit. Our daily menu is also available on the Ursuline website.
