Transcripts can be obtained through the Guidance Office.
 
 

Requirements for obtaining a transcript include:

 

1) Graduate’s request in writing:
            a) in person
            b) via U. S. Mail (see address below)
            c) via fax (513.791.3501)
            4) via email: jfoertsch@ursulineacademy.org

2) Graduate’s date of birth
 
3) Year of graduation
 
4) Address or fax number of recipient
 
5) Full name at graduation
 
6) $3.00 fee for each transcript to:
            Guidance Office
            Ursuline Academy
            5535 Pfeiffer Road
            Cincinnati, OH 45241
